OVERALL JOB FUNCTION
Under the supervision of the Centre Manager and Clinical Manager, the Linkage officer will promote healthy behaviors and prevent risk behaviors in AGYW and support eligible AGYW in accessing relevant services. The linkage officer will link all the AGYW/ABYM in the program for both Biomedical and CORE services. Working closely with the Biomedical team and the PGT’s from the SSR’s to ensure successful referral of AGYW to CORE services. Ensuring successful linkage to care for all AGYW and adherence counselling.
RESPONSIBILITIES
Develop and implement activities to encourage health seeking behaviors among AGYW in school and out of school
Ensure quality adherence counselling at all AGYW program service points (Safe space, satellites, Mobile clinic)
Monitor access and uptake of HTS and linkage to care to AGYW friendly services at sub district level.
Ensure that the NDoH Adolescent and youth-friendly services (AYFS) standards are implemented and monitored at AGYW prevention program sites.
Provide guidance to the program peer educators on delivering HIV services to AGYW in-school and out of school.
Work closely with schools and satellite sites on monitoring linkage to services of AGYW
Ensure all records of activities are well managed to ensure effective data collection.
Identify and establish working partnerships with community stakeholders focusing on AGYW at sub-district level.
Working closely with the team leaders and PGT’s from the SSR to ensure successful referral of AGYW to CORE services.
Adherence Support groups to ensure adherence to treatment by AGYW’s.
Tracing of loss to follow up of clients of ART, PrEP and other SRH services to ensure continual care.
Identifying needs of the AGYW’s in and out of school and refer for appropriate services.
Monitoring of AGYW’s on treatment and early detention of loss to follow up to avoid defaulting of clients.
Facilitation of support groups in the facilities e.g., Youth Care Clubs.
COMPETENCIES
Excellent verbal and written communication skills in both English and Zulu.
Experience in managing and building capacity (i.e., training, mentoring, coaching and supportive supervision) of fieldbased teams, including facilitation skills.
Adherence Counselling Skills
Ability to take initiatives, work under pressure and multitask with minimum supervision.
A team player with good interpersonal skills.
Sound problem solving and decision-making skills; effective time management and coordination skills.
Competent in Microsoft Office suite will be an added advantage.
QUALIFICATIONS & EXPERIENCE
Grade 12/Matric Certificate.
HIV Counselling and Testing Certficate.
Experience in working as a linkage officer is highly recommended.
Qualified HIV Counsellor with preferably an experience in working with AGYW or an NGO.
Experience with public speaking and trained on facilitation skills is recommended.
Have knowledge and experience in TB/HIV/AIDS management.
Excellent communication, interpersonal and supervision skills.
Excellent administrative and organizational skills.
Proficiency in MS office is recommended.
Strong organizational skills, including time management and multi-tasking skills.
In-depth understanding of public health system especially in KwaZulu-Natal province
Ability to travel within uMhlathuze sub-district and King Cetshwayo District
A Valid Drivers’ license and ability to drive will be an added advantage.
